Provider Identification Number

The Department of Health Services assigns each provider a unique nine-character identification number which identifies the provider through a claims processing system and enables the provider to receive payment. Separate provider numbers may be assigned for inpatient and outpatient claims submission. Sometimes known as a Physician Identification Number.
